Questions about Sign Letter
Notary signed letter to verify seperation?
I am trying to renew my sons medicaid and since the last application (last April), my husband and I have seperated, but haven't filed any paperwork yet. The DHHS needs a notarized letter to this fact. I know some notaries, but not sure how to word the letter.
Has anyone else had to do this before?
I haven't had to do this but I suggest simply saying something like
This is to certify that on _______(date), by mutual agreement, (Husband's Name) and (Wife's Name) are living separately and apart.
Then put a line for your signature, a line for your husband's signature, and sign and date the form before a notary.
You may also want to ask DHHS if there is any specific language they require. If not, I'd try the above. Good luck.
